Summary: A healthy sexuality plan is a written document that describes:
- The behaviors the patient wants to change
- The high-risk behaviors
- Behaviors that are supportive for sexual health (for example, physical exercise or relaxation techniques). General lifestyle modifications, such as a balance between work and recreation, should also be considered.[1]

The Boundaries column is a list of rules the client is ready to accept, e.g. I will not watch porn.
The Ambivalence column are things that the client is not yet sure he is ready to change, but he is ready to monitor them, and decide in the future whether to move them to the Boundaries column. E.g. I will monitor my masturbation frequency for the next 30 days and then decide if I want to stop or reduce it.
The Health column is for actions that the client can take. E.g. I will install a filter.
